There are two ways to pre-register.
1.Complete the pre-registration form and send to the below address.
2.Send an email to jpniemuth@idcnet.com with the information from this form. Include the subject line “FOTP Golf Outing Pre-Register”.
Pre-payment can be sent with pre-registration. Checks can be made payable to “Whitewater Booster Club”
